Salary Range:£43,000 – £48,000 plus bonus entitlements

About The Role

The Nurse Case Manager holds primary responsibility for the development, implementation, and delivery of care plans and clinical interventions for an allocated caseload of clients. The ideal candidate will work closely with clients, their families and other multi-disciplinary healthcare professionals to provide top quality client-specific clinical advice and guidance to care delivery teams, training and supporting them with their clinical skills and associated activities.

Successful candidates will hold active NMC registration to work with children.

Additionally, the Nurse Case Manager will complete routine observations of care delivery within care settings, including outside of normal working hours, to uphold the company’s standards and expectations in all aspects of care.

Duties will include:

  • Managing a case load of clients, acting as the main point of contact for any clinical updates, developments, or changes in need.
  • Involvement in assessment of prospective care packages and development of support frameworks.
  • Responsible for development, implementation, and analysis of all care plans, risk assessments and client focused documentation for clients within their caseload.
  • Conduct regular reviews of all client care plans and documentation
  • Responsible for the safe management and administration of medication within care settings.
  • Safeguarding children and vulnerable adults from risk of harm or abuse, and escalating concerns to the Company’s Designated Safeguarding Officer for consultation where required.
  • Monitoring, supporting, coaching, and induction of care staff.
  • Providing on call support to services outside of normal working hours, including weekends and bank holidays, through participation in the on-call roster.
  • Actively promote best practice in all elements of care delivery, leading by example, and providing coaching and mentorship to develop the skills, knowledge and confidence of care staff.
  • Conduct routine and unannounced visits to care locations to support the company’s objective to provide high-quality care through effective monitoring and observations.
  • Liaise with external professionals, commissioners, and multi-disciplinary teams who have input into the care of the individual.
  • Identification and delivery of training for the service that will enhance the quality of support provided to the service user.
  • Cooperate with the company’s PHB training strategy, including through the delivery of high-quality training to external parties.

About You

Successful applicants will have:

  • active and unrestricted registration with the Nursing & Midwifery Council
  • experience of paediatric care
  • experience of community healthcare

You will also ideally have experience in the delivery of clinical training and completing competency assessments to support the application of clinical skills to complex care.

You will be as keen to help as you are keen to learn, and will have a positive can-do attitude when working within the team.

As you will be visiting our staff and clients, you will need to have a Full UK Driving Licence, access to your own vehicle, and the relevant motor insurances.
