Referral Process

At Unity Care Solutions, we understand that every individual who requires care is unique and we ensure that this is at the forefront of everything we do. This starts from the first point of contact and continues throughout the referral and start up process.

1. Initial Enquiry

At Unity Care Solutions we work with private clients as well as Social Services and Clinical Commissioning Groups. This means you can contact us direct. You do not need to be referred by a doctor, hospital or other healthcare agency. Whether you contact us by telephone or email, we will discuss with you your immediate needs and the type of care you are looking for.

2. Initial Quote

The initial quote is provided to give you an indication of the care package costs based on the information provided within the initial enquiry.

3. Initial Care Needs Assessment

The next stage is for one of our specialist team from your local office to come and meet with you to complete the initial care needs assessment. The assessment might take place at home, or perhaps in hospital. They will ask a lot of questions about health, mobility and other relevant aspects of daily life. The purpose of this meeting is to find out all the information we need to be able to provide the highest quality care that meets your individual needs. We will work with you to set outcomes and goals and also encourage and welcome involvement from the individual and their family/advocates where appropriate and available.

4. Identification & selection of suitable nurses or care workers

This part of the process is very important. You will have a close relationship with your care team, so we work with you to make sure we select the right people. Following the initial care needs assessment, the office team will look to identify which members of our highly skilled staff team would be best suited to meet not only your medical needs, but also your emotional needs and overall wellbeing. All staff at Unity Care Solutions come with various care backgrounds and previous verified experience. Following successful interviews, our applicants undertake an enhanced DBS (criminal record check), undergo referencing and employment history checks and all staff complete our detailed induction and training programme. This ensures we are able to provide you with the highest quality nursing and care staff to build your team that will offer you continuity and consistency in your care package.

5. Care Plan

Following the initial care needs assessment, we will start developing your bespoke care plan document along with all relevant risk assessments and protocols needed to ensure the staff team can deliver the right care for you at all times. With your permission, we will liaise with the other professionals and specialists that are important to you and that are involved in your care needs to ensure we are working collaboratively and seamlessly to deliver the highest possible standards and maintain safety and dignity for you at all times.

6. Meet The Team

Once we have identified staff members that we feel would be right for you, we will arrange a ‘meet & greet’ visit with each of them. This will give you the opportunity to put a face to a name and start getting to know each other. We will then discuss this visit with you to gain feedback and will also speak with the staff members too. We feel it is important that the matching process is right for both the client and the staff member from the start to ensure a positive and professional relationship is built based on the right foundations.

7. Client specific training

After the staff team is confirmed, one of our nurse case managers will arrange any client specific clinical training that may be required. This ensures that your team adapt their current skillset to be able to meet all your care and medical needs in a way that is right for you.

8. Ongoing Case Management and Support

Once your care plan has been agreed and the package of care commences, you will have a dedicated co-ordinator and nurse case manager who will look after you and the staff and oversee your full package of care on an ongoing basis. We carry out regular reviews, ensure your care plan remains current and up-to-date, manage the staff and rota’s and can also support you in accessing other services if or when required. In the early stages of the care commencing, the office team will have more frequent contact with you to ensure all your questions are answered, any teething problems are addressed promptly and that the care is working well. We also have a 24/7 on call service to ensure that there is always someone on hand to help.
