So, what is Complex Care?
Essentially, complex care (also known as Continuing Heath Care) is when an individual is managing significant and ongoing healthcare issues such as chronic illnesses and disabilities. People with complex care often have multiple health conditions which means delivering personalised bespoke care is of the highest importance.
We understand how important it is to manage the physical and mental health conditions diagnosed, but it is equally important to take into account the experience a client is having and taking a holistic approach is essential. Our overall goal is to enhance quality of life, enable individuals to reach their full potential and to live independent and fulfilling lives.
So, what is Complex Care?

Building YOUR complex care package:
Throughout the building of your complex care package and the recruitment of the team, we will keep you as informed as you would like to be. We will always arrange meet and greets with you with potential team members prior to assigning them to your package of care. We truly believe that open and honest communication is key to a successful relationship. Once your team members are identified and fully checked in line with CQC requirements and our professional duty of care, we will then provide any training required and ongoing support for the team. Training will be classroom and home based to ensure it is fully personalised to your needs. We will continue to monitor the care delivery throughout and ensure we are on hand to help when needed. Your Nurse Case Manager can visit you as often or as little as you like.
In addition to supporting you and your team, we can also provide evidence on a regular basis, or as and when required, in any format for case managers or commissioners.
Care packages built around you:
We work closely with the NHS Community Teams, CCG’s, local hospices, Dieticians, Occupational Therapists, specialists/consultants and other health professionals to help co-ordinate the care that is required. Your care package will be built around you and is designed to meet your individual needs. The care delivered can be at home, at work, on holiday, at nursery, school, college or university or anywhere else you need it to be. We can be engaged at any stage of your journey, from the first point of discharge home from hospital or at a later date if preferred.
We welcome any complex care enquiries and will be honest in what we can and cannot deliver. From the initial assessment, always carried out by an experienced registered nurse, we will start to design your complex care package with you. Considering your likes, dislikes, wishes and aspirations.
What care can Unity Care Solutions provide:
1:1 or 2:1 complex care delivered by Registered Nurses or trained Healthcare Assistants
Up to 24 hour round-the-clock care if required
Specialist and bespoke complex care packages
Waking night or sleeping night support
University, work or educational support
All care packages are assigned a dedicated Nurse Case Manager and Care Co-ordinator to help manage and oversee the day-to-day care needs
